This collaborative Guideline on endoscopy in patients on antiplatelet or anticoagulant therapy updates the 2016 Guideline from the British Society of Gastroenterology (BSG) and the European Society of Gastrointestinal Endoscopy (ESGE). Evidencebased revisions have been made in the stratifications for the risk of hemorrhage associated with given endoscopic procedures and the risk of thrombosis associated with discontinuation of antithrombotics. A more detailed risk analysis for atrial fibrillation has been employed, and the recommendations for direct oral anticoagulants have been strengthened on the basis of recent trial data. There is a new section on the management of patients presenting with acute GI hemorrhage and important patient considerations are highlighted. The balance of risk between thrombosis and hemorrhage in given situations remained key!
