UPPER GI ENDOSCOPY

(Therapeutic)

Complications of esophageal laser therapy. An overview

Direct laser complication:

thermal injury

Indirect laser therapeutic complications:

endoscopic complications
coaxial Co 2 inflation

Typical complications of esophageal laser therapy endoscopy-related

Perforation

Induced active bleeding

- Mallory-Weiss tear,
- trauma of esophageal varices

Bleeding post laser therapy

Bleeding post Nd Yag laser coagulation by the destruction of small neoplasic vessels. Mild haemorrhage

Hypotension

Aspiration

Acute - tissue erosion

Subacute - tissue necrosis

Chronic - fibrotic scarring

Acute thermal injury induced by laser treatment

results in complications up to 12 hours after procedure :

Induced haemorrhage

Acute perforation

Subacute thermal injury induced by laser treatment

results in :

ulceration

delayed haemorrhage

delayed perforation

fistula

fever, pain

Chronic thermal injury induced by laser treatment

results in :

stricture

obstruction - following hemostatic laser treatment

re-obstruction - following tumors laser treatment

Typical aspect of a neoplasic stenosis by a tumor growth post laser therapy, this lesion is located at the middle part of oesophagus.
